 H1N1 (swine flu) Vaccination Clinics 

The Centers for Disease Control and Prevention’s (CDC) Advisory Committee on Immunization Practices has recommended that children and young adults aged 6 months through 24 years be vaccinated against the 2009 H1N1 influenza virus as soon as possible. The Perth Amboy Public Schools are working in conjunction with the Middlesex County Health Department to give parents and/or guardians the option of having the 2009 H1N1 influenza vaccine available to your child at school. There will be no charge for this vaccine.
Please review the following forms:

                        1.)  Vaccine Consent Form
                        2.) “Vaccination Information Statements”
                              Inactivated (the Flu Shot)/Desactivada (inyección contra la influenza)
                              The Nasal Spray Vaccine/rocío intranasal  

             Adhering to CDC recommendations, we have scheduled preschool vaccination clinics first, followed by elementary school clinics, middle school and then the high school. A letter will be sent home with your child, with the date of your schools clinic, once we have obtained vaccines from the Middlesex County Health Department. If you would like your child to receive the vaccine, bring the completed and signed vaccine consent form
with you when you take your child to receive the vaccine. Please bring your child to his/her school.
